Expert analyses

The expert analysis “Collected data concerning birth and neonatal health and mortality in the Czech Republic” was written in 2013 for UNIPA and updated in 2014, in reaction to repeated desinterpretation of missing data in ÚZIS (Institute of Health Information and Statistics) database in comparison to ČSÚ (Czech Statistical Office) database. It summarizes the way data about pregnancy, birth and the newborn are collected and analysed. Several aspects of the differences in databases are discussed. Common media myths are refuted with help of the collected data. Several foreign and international perinatal databases are presented and commented on. The updated version with many links and relevant documents is published here on the blog. Available in Czech only.

The expert analysis “An overview of recent studies examining the midwife-led care” was written in 2012 for the project Normální porod (Normal birth) and the Maternity Care Workgroup of the Ministry of Health of the Czech Republic. It gives a succinct, comprehensive summary of four most important recent studies examining the primary midwife care, midwife-led care and homebirth care. Available in Czech only.

Articles in paper and internet media

December 23rd, 2013 An article entitled “Let’s take a healthy care of children and mothers!” was published in the printed version of Lidové Noviny newspaper. The text is a reaction to the press release of the Ministry of Health stating that the Ministry will permit early leaving of maternity hospitals after birth and allow for midwife-led care in the maternity hospitals, starting January 2014. I point to the fact that this novel arrangement – presented as revolutionary in the media – is a mere cosmetic and non-functional modification to present state of things. As the most often used argument against midwifery care are the excellent Czech perinatal results, I show the readers using the data from Sweden that there is no need for fear of changes in Czech maternity care – both the scientific evidence and the Swedish model show that the primary care led by a midwife is a safe and healthy option for childbearing women and their children.

January 4th, 2013 An article entitled “Let’s give up the eugenics”, written together with the clinical psychologist Michaela Mrowetz, was published in the printed version of Lidové Noviny newspaper. It’s a shortened version of the text “Juliana’s doll” published elswhere. We have been inspired by the documentary “Save Edwards” directed by Dagmar Smržová about the decision-making process and the life of the Sladkowský family and their oldesr daughter Juliana. We comment on a steep increase of TOPFA (termination of pregnancy for anomalies) for this and other diagnostic reasons due to more extensive and more precise prenatal diagnostic techniques and hence on a steep increase of traumatic events in the reproductive history of Czech women.

February 3rd, 2012 In the internet newspaper Deník Referendum I have commented on poor work of Czech journalists concerning two homebirth topics: the newly published Birthplace in England study and the heart-attack caused death of Australian homebirth aktivist Caroline Lovell.

November 21st, 2011 A polemic text published on web portal Zdravotnické noviny (Medical news). It’s a reaction to a published opinion of Alena Měchurová, MD, and the Perinatology section of the Czech Gynecological and Obstetrical Society statin that “All contries that tolerate homebirth have worse perinatal results than the Czech Republic, in spite of tha fact that only low-risk women give birth at home”. In my polemic I refutate this and discuss the fact that in the Czech Republic we still use the old definition of stillborn child (with birthweight 1000g and higher) while the EU countries use the same definition as for the live born child. In reaction to this article I gave an interview for the web portal iDnes.cz that was published with the title “Neonatal mortality not so low as the Czechia claims it to be”.

December 6th, 2010 A polemic entitled “Objective data about dead babies” was published in internet news portal Zdravotnické noviny (Medical News). Together with clinical psychologist Michaela Mrowetz and lawyer Adéla Hořejší we refutate a widespread media myth about 5-6 dead babies each year due to homebirth, a myth that was created and repeated by many Czech obstetricians. We show that these assertions are extremely manipulative as they use data from all out-of-hospital births, irrespective of the pregnancy term or planning status.

December 12th and 22th, 2008 Two-page article in December double-issue of printed Zdravotnické noviny entitled What should be (super)standard in maternity care?. It is a reaction to a current discussion about possible division of health care to standard and superstandard (i.e. directly payed without refund). It was thought that while the use of epidural analgesia should be considered as standard care, the use of water immersion during birth or home visit of s midwife before and after birth should be considered as superstandard and non refundable. I use evidence-based findings published in Cochrane Summaries to show that many standard and cheap interventions (e.g. routine episiotomy, routine amniotomy, enemas) lead to higher costs of care while not helping better health results.

Public talks and causes

December 9th, 2013 Guest at Debate of the Republic Salon organized by sociology students from University Hradec Králové. In the Oxford-style debate the discussed thesis was “Is hospital the only safe place for birth?”. The proponents were Leoš Heger, the former Minister of Health and Marian Kacerovský, the lead physician of the obstetrics unit of the University HK Hospital. The oponents were Adéla Hořejší, the lawyer and Markéta Pavlíková, the biostatistician. My seven minutes opening talk was centered around the fact that it is not the place of birth but the type of care that makes the birth physically, psychologically, socially and spiritually more healthy.

September 10th, 2013 I spoke as a biostatistic expert on behalf of the claiming side at public hearing of the case Dubská and Krejzová vs. The Czech Republic at Euroean Court of Human Rights in Strassbourg. The claimants sued the Czech government that failed to provide the possibility of professional care during an out-of-hospital birth.

January – December 2012 I was a member of the Maternity Care Workgroup set by the Minister of Health Leoš Heger. Together with Adéla Hořejší, lawyer, Marie Vnoučková and Radmila Dorazilová, midwifes, and Monika Šimůnková, lawyer and Government Deputy for Human Rights we submitted various study material, statements of respected international organisations, information about evidence-based care, proposed regulations for the independent midwife work. We also participated at the resolution leading to acknowledgement of the independent midwives work and its incorporation into the state healthcare system, with stress to good education and licesing criteria. However, this resolution was refuted by obstetrician Workgroup members without any discussion.

March 27th, 2012 Talk at one-day conference „Homebirth – a step forward or back?” organized by the Health Commitee of the Chamber of Deputies of the Parliament of the Czech Republic. The talk showed the scientific evidence that the midwife-led care is safe provided it is an organic part of the healthcare system.
